If you are scheduled for an evaluation, please print and fill out the forms that pertain to your child to bring to your scheduled appointment time.

To Schedule a Visit:
To schedule a visit at our facility, we will need the following items:

Referral from your child's primary care physician

Insurance information

Details about your areas of concern
Once You are Scheduled:
Once your child's visit is scheduled, we ask that you:

Fill out the appropriate history form (see below)

Read our Notice of Privacy Practices

Send any pertinent medical history/previous testing to us that is pertinent to the areas of concern for your child
Insurances Accepted:
We do accept some state and private health insurances as well as other scholarships and grants.
Please call our office to determine whether or not we are providers for your child's insurance.
If you have a private insurance plan, you should first contact your child's insurance company to determine benefits of your particular plan.
